The required forms to play a sport at Sacred Heart are available for download below. Athletes will also need a sports physical before they play their first sport at Sacred Heart and it will be valid for the entire school year - Fall, Winter and Spring Sports. All participants are required to have a sports physical before each school year and before the first practice begins. Most doctor's offices have the required sports physical form on hand. Ask for it at the time you schedule or show up for your appointment. All physicals must be dated on or after April 15 for the current year.  For example, for the 2024-2025 season, the sports physical must be dated on or after April 14, 2024.

Concussion Awareness Sheet and Acknowledgement Form. Please read the material and sign the form. This form is only required once for the entire playing career for an athlete in the Sacred Heart CYO program.  We are required to keep these forms on file.
